How to Recover From an ACL Injury: A Realistic Timeline

What each phase of ACL rehab actually involves, the milestones that matter, and why the calendar is the worst way to decide when you play again.

The first two weeks decide the next six months

Full knee extension and a working quadriceps are the two things that predict a smooth ACL rehab. Athletes who lose extension early spend months chasing it later. Swelling control, extension drills and quad activation matter more than any fancy exercise at this stage.

Strength before speed

Running is not a reward for time passed — it is earned with quadriceps strength, single-leg control and a swelling-free knee. Most athletes are ready for straight-line running somewhere between month three and month four, provided strength testing supports it.

Return-to-sport testing is not optional

Limb symmetry above 90% across strength and hop testing, plus sport-specific conditioning, is the minimum standard before clearance. Returning before nine months significantly increases re-injury risk in pivoting sports.
